For me, the most important difference is psychologically. On a demo account, a trader does not work with his own money.

You are using virtual money which you can spend without worrying about anything, so it is more of a training for a real thing. As soon as you switch to trading with real money, then you start getting anxious and try to make decisions very carefully.
I started with 3 months of demo on strategy profitable by end of 3 months. Went live, started well, but once size up my trades in live trading everything went south.
Now I am back on demo, and the previous live experience in live trading has made me look at demo account the same way as it does with live, emotions comes into play even when trading on demo.
This time I hope to get better results on demo, size up on demo too and deal with the mental issues and still profitable go back to try live trading again.
